About the Health and Wellness Center:
Harbor Care Health and Wellness Center offers primary, preventive medical services, behavioral health services, and dental services to children and adults who are homeless or at risk of homelessness, uninsured, under-insured, or low income.Harbor Care offers a sliding scale fee for self-pay patients, and accepts Medicaid, Medicare, as well as most commercial insurances. The Southern NH HIV/AIDS Task Force has a long history of providing culturally sensitive support services to low-income individuals living with HIV/AIDS. With medical case management at the heart of our program, each client is assisted in creating a service plan with action steps leading to viral suppression while meeting their other basic needs.
Job Summary:
The Per Diem Pharmacist will partner with Harbor Care Pharmacy staff to assist customers with their health care needs by filling prescriptions, providing clinical services, including but not limited to immunizations and MTM, and excellent customer service while managing Pharmacy associates.
Frequent independent judgments are essential. The Pharmacist will work in collaboration with health center Providers to ensure adherence to the strict regulatory climate, and report to the Pharmacy Director.
Basic Qualifications:
Active Pharmacist license in good standing with the New Hampshire Board of Pharmacy.
Immunization Certification through an accredited organization and endorsed by the Board of Pharmacy.
Preferred Qualifications:
Experience as a Pharmacist in a community pharmacy.
Ability to work a flexible schedule to meet the needs of the business.
MTM certification and experience will be preferred, but not required.
340B knowledge and experience will be preferred, but not required.

Essential Duties and Responsibilities
The Pharmacist is responsible for the functions below, in addition to other duties as assigned:
Ensure the accuracy and appropriateness of all prescriptions filled by completing Drug Utilization Review and Final Quality Assurance, applicable to state and federal Board of Pharmacy regulations.
Build profitable business and script growth through recommended clinical programs including appropriate immunization and when available MTM (Medication Therapy Management), Collaborative Pharmacy Agreement (CPA), and all ongoing other programs as identified.
Provide excellent customer service by assisting customers with medical and pharmacy related issues and providing healthcare counseling.
Proactively immunizing and completing MTM interactions as directed.
Supervise the work completed by support staff while on duty.
Assist with training, coaching and managing the pharmacy associates and assume management responsibility over the entire pharmacy, as needed.
Maintain appropriate security of the Pharmacy department.
Comply with HIPAA and all other applicable federal and state laws and regulations.
Handle and resolve customer issues, complaints and questions to build customer trust and loyalty.
Interact with physicians, APRNs, PAs and utilize reference material to gain information on customers and prescriptions and to resolve any issues that arise.
Assist with maintaining the Pharmacy department by keeping it clean and in order.
Prioritize Pharmacy Department tasks and follow through to ensure all tasks are completed in a timely manner. Including, but not limited, to cycle counts, inventory management and will-calls to customers.
