View Point Health is a community behavioral health center utilizing a team of psychiatrists, nurses, licensed counselors and social workers, as well as other professionals to provide services to individuals who need treatment and support to cope with mental illness, substance abuse, and intellectual and developmental disabilities. View Point Health serves uninsured, underinsured, low-income Medicaid, Medicare, war veterans and some private insurance across multiple locations with a full continuum of behavioral health services and support.
Intensive Case Management (ICM) uses a multidisciplinary team approach to provide intensive mental health treatment services to individuals with severe and persistent mental illness in their homes and other community settings-24 hours a day, seven days a week, and 365 days a year. ICM is intensive, and its goal is to keep individuals out of state hospitals, jails, emergency rooms and other institutions. ICM clients have to meet a strict criterion (i.e., diagnosed with a severe and persistent mental illness, unable to maintain activities of daily living, and be unsuccessful in other forms of treatment). ICM clients often have no resources and no family support and require an intensive and expeditious form of treatment.
Duties & Responsibilities:
Completes diagnostic formulations and appropriate assessments on clients with mental illness and substance use disorder.
Collaboratively formulate a treatment plan addressing issues identified in assessments.
Provide short-term individual and group therapy using evidence-based practice therapies for client with SPMI &/or SUD
Assist clients in identifying, developing, and sustaining appropriate natural support systems.
Provide psychoeducation around mental health, substance use, trauma, and recovery.
Collaborate with other behavioral health and physical health providers to holistically address client needs.
Complete all required documentation (daily clinical notes, assessments, treatment plans) in an accurate and timely manner (agency standard 24hrs).
Participate in clinical supervision to increase competence and professional development.
Participate in on-call rotation to provide after hour coverage, via phone.
Completes annual training requirements as outlined by agency and/or supervisor.
Performs other related tasks as assigned by the supervisor.
Other duties as assigned.
Minimum Qualifications:
Master's degree in a related area from an accredited college or university AND eligibility to be licensed AND Two years of supervised experience in the area of assignment.
Note: Licensed as an LAPC, LPC, LMSW, LCSW, (associate or full licensed required)
Preferred Qualifications:
Experience with Carelogic or other HER/EMR.
Previous clinical experience working in Community Mental Health Care with SPMI, SUD, &/or Co-Occurring Disorders.
Experience providing ASAM or substance use specific groups such as Dual Diagnosis, DBT, Seeking Safety.
Requirements/Competencies:
Must be an associate licensed or fully licensed clinician APC, LMSW, LPC or LCSW in the state of Georgia.
Must be able to lift 20 pounds.
Requires long periods of sitting/standing.
Must have valid Georgia driver's license and Motor Vehicle Record in accordance with company policy.
Candidates for selection must pass a criminal background check (including fingerprinting).
Must pass a pre-employment drug screen and subject to random drug screens.
Any combination of training and experience, which would have enabled the applicant to acquire the necessary knowledge, skills and abilities.
Note: Some positions may require a valid driver's license.
