Behavioral Health Case Manager - Dekalb/Fulton - ETP Project Light (Personal Services)

Early Treatment Program is a team based, multi-element approach to treating transition age youth and young adults, ages 16-30 years old, with FEP (First Episode Psychosis). This position will be based out of our DeKalb Office. The team of professionals will include a Psychiatrist, Registered Nurse, Employment Specialist, Clinical Team Leader/Individual Therapist, Case Manager, Care Coordinator/Certified Peer Specialist, & Family Peer Support Specialist.
CSC (Coordinated Specialty Care) is a collaborative, recovery-oriented approach involving consumers, treatment team members, family members (when appropriate), as active participants. CSC emphasizes shared decision making as a means for addressing the unique needs, preferences, & recovery goals of individuals served. CSC services are also highly coordinated w/ primary medical care, w/ a focus on optimizing consumers overall mental, emotional, & physical health.
Care Coordinator/Peer Specialists strives to help people reach their own goals and live healthy lives by supporting them in identifying strengths and abilities followed by assisting them in articulating, developing, and creating personal goals for recovery through one-to-one and group sessions. Peer Specialists support the therapeutic milieu and assist consumers in accessing therapeutic and ancillary services.
Care Coordinator/Peer Specialist assists clients with daily decision-making and minor problem-solving skills. Provides emotional support and encouragement and models appropriate behavior and social interactions. Observes and monitors participants' behavior during daily activities. Intervenes and diffuses potentially disruptive situations using established guidelines. Refers concerns to appropriate staff as needed. Charts observations and incidents. Assists with evaluation of functional levels of participants. Works with other team members to determine appropriate individual goals and objectives and to develop individualized plans for participants.
Duties & Responsibilities:
Interviews client, family members, educational institutes, and/or support system(s) to assess level of functioning, current Strengths, Needs, Abilities, and Preferences and secure information relative to program requirements for receiving services and recommending appropriate level of care.
Participate in weekly treatment team meetings to assist in the development of treatment plans with appropriate therapeutic interventions, notifies professionals of any unusual observations or incidents, client progress, and follow up.
Manages caseload up to 30 clients & monitors consumer progress & compliance with their service plans (treatment plans)
Report concerns and needs of the program or clients to Team Lead. Recognize the need for crisis intervention and notifies supervisor and professional staff of escalated situations.
Develop & Sustain community partnerships (including local high schools, colleges, educational institutes, & youth programs)
Facilitates referrals and linkages to community services and communication among client's service providers with VPH to assure a continuum of services.
Provide linkage, transport clients, and assist/coach clients in accessing & obtaining community resources.
Write precise, accurate, and clear progress notes in our electronic Care Logic system in a timely manner. Maintain required records & information in each client's file. Prepares accurate, complete and timely documentation of case notes and billing information to meet agency and state standards and provide effective service delivery across a broad range of services.
Provide individual skills training as indicated in the client's treatment plan including but not limited to independent living skills, coping skills, interview skills, symptom management skills, anger management skills, assertiveness skills, advocacy skills, hygiene skills, and many other daily living skills.
Establishes linkage with community resources (jails, hospitals, shelters, school, DFCS, etc.) and to assist with transition, discharge planning, and/or a smooth transition to View Point Health and other services.
Under professional guidance, provides individual and group supportive counseling to clients and families concerning their needs and problems, and issues with personal and social development.
Attend interdisciplinary meetings, trainings, and continuing education relation as it relates to the job position
Minimum Qualifications:
Bachelor's degree in a related field from an accredited college or university AND At least 1 year of experience working with youth and young adults at transition age at least 1 year of experience working with SPMI (Severe Persistent Mental Illness)
Preferred Qualifications:
Bachelor's degree in social services or closely related field from an accredited college or university.
Knowledge of community resources.
Ability to work independently and as a team member.
Good verbal and written skills
Must be a current or former recipient of behavioral health services, with Certification as CPS.
Good communication skills
Good time management skills
Consistent and reliable attendance
Requirements/Competencies:
Must be able to lift 20 pounds.
Requires long periods of sitting/standing.
Must have valid Georgia driver's license and Motor Vehicle Record in accordance with company policy.
Candidates for selection must pass a criminal background check (including fingerprinting).
Must pass a pre-employment drug screen and subject to random drug screens.
Any combination of training and experience which would have enabled the applicant to acquire the necessary knowledge, skills and abilities.
Note: Some positions may require a valid driver's license.
