Behavioral Health Clinician (GLS) Rockdale/Newton (Personal Services)
View Point Health proudly provides outpatient treatment to children, adolescents and adults. At the outpatient centers, we promote overall health and quality of life for individuals, youth and families through evidenced-based therapies and medication management to reduce the impact of mental illness, behavior challenges, and substance use.
The Clinician will work as an integral team member to provide support to youth and young adults ages 10 to 24 within the framework of the Garrett Lee Smith grant and Zero Suicide models, among others.
Through community outreach and education, the youth clinician will increase the number of youth, in the local community that have a behavioral health home, as well as support screening of parents/guardians, parenting education and training in the local community. The youth clinician will work with referring agencies such as schools, child welfare, juvenile justice systems, local health departments, community health centers, non-profits, afterschool programs and any other youth serving agencies in the catchment area. The clinician will be responsible for quality assurance efforts in treatment services as well as coordinating the delivery of patient centered, culturally and linguistically appropriate care and training to staff.
In addition, youth and their families will receive counseling on access to lethal means. Providers will develop a suicide safety plan with clients and regularly monitor the individuals electronic health record to identify treatment adjustments. The plan will include rapid follow up for any discharge from hospitals and other treatment facilities of youth that attempted suicide, are in suicidal crisis, and/or have severe mental health illness. The safety plan will also include lethal means assessment, information resources and protocols to ensure patient safety. Clients identified as high risk for suicide will receive an outreach telephone contact within 24-48 hours post discharge and will receive an appointment for follow up care within one week. Emphasis will be placed on providing the treatment and support that is most appropriate to their needs. Provides counseling/therapy to clients requiring complex intervention.
Duties & Responsibilities:
Perform program functions in suicide prevention evidence-based counseling, outreach, intervention, and case management for youth ages 10 ~ 24.Provides biopsychosocial and diagnostic assessments on new, existing clients and/or hospital discharges for those experiencing thoughts and behaviors related to suicide.Conducts and assists with crisis interventions and collaborating with local hospitals for treatment.Work with clients to develop SMART and recovery-based treatment plans.Assist clients in identifying, developing, and sustaining naturalsupport systems. Provide psychoeducation to clients and families around mental health, substance use, trauma, recovery and access to lethal means. Collaborate with other behavioral health and physical health providers to holistically address client needs.Participate in regular team meetings to collaborate regarding shared clients, build and practice new skills, and stay up to date on agency administrative information.Participate in clinical supervision to increase competence and professional development.Completes annual training requirements as outlined by agency and/or supervisor.Travel is required in Newton and Rockdale counties in your own vehicle to meet with youth and young adults at home and community locationsWork hours may vary depending on client schedules and needsAttend weekly team meeting to identify and address additional client needsReceive specialized training in order to facilitate trainings or groups to clients and / or community partnersMaintain accurate, timely and detailed documentation to meet VPH, state and federal grant reporting needsMaintain credential through ongoing state sponsored professional development activities and trainingsOther duties may be assigned
Minimum Qualifications:
Master's degree in a related field from an accredited college or university AND Fully or Associate licensure (LCSW/LPC/LMSW/LAPC) in the state of Georgia.
Preferred Qualifications:
Experience with Carelogic or other EHR/EMR Previous clinical experience working in Community Mental Health Care with SPMI, SUD and / or Co-Occurring Disorders Experience providing suicide specific evidence based individual and group treatmentKnowledge of local community partner systems
Requirements/Competencies:
Must be able to lift 20 pounds.Requires long periods of sitting/standing.Must have valid Georgia drivers license and Motor Vehicle Record in accordance with company policy.Candidates for selection must pass a criminal background check (including fingerprinting)Must pass a pre-employment drug screen and subject to random drug screens.Any combination of training and experience, which would have enabled the applicant to acquire the necessary knowledge, skills and abilities
